Femoral Vein-Femoral Vein VV-ECMO-Assisted Anesthesia for Giant Retrosternal Goiter Surgery: A Case Report
Abstract
Background: Managing giant retrosternal goiters that cause severe airway stenosis and vascular compression presents significant challenges for anesthetic teams. This case report describes the anesthetic management of a patient undergoing resection of a giant retrosternal goiter with femoral vein-femoral vein venovenous extracorporeal membrane oxygenation (VV-ECMO) support.
Case presentation: A 51-year-old woman presented with a two-year history of anterior neck enlargement and progressively worsening dyspnea over a period of seven months.
Preoperative imaging revealed a right thyroid lobe measuring 57 × 57 mm, extending into the mediastinum and compressing the trachea to a minimum diameter of 2.46 mm. A multidisciplinary team recommended thyroid resection with VV-ECMO support. The procedure was successfully completed, and ECMO was weaned postoperatively. The patient was discharged on postoperative day 15, experiencing no major complications.
